If you go through the online application, you'll see that there's an option to upload a digital photo taken with your phone. In that case no 'code' is needed.

About 4 years, 1986-1990. I said it was a long time ago! I was a US resident the whole time, on a green card. Moved to Pennsylvania for work, then moved back.
