It really depends on how good your medical insurance is. Most US medical insurance policies require that you pay a portion of the fee to visit the doctor. If memory serves I think I paid $10 to visit...

My wife who is an American told me that to get the grade just deduct 5 from the childs age. A 6 year old would be 1st grade, a 9 year old would be in the 3rd grade.
