You can scan and upload your own documents or have them scanned and uploaded at the appointment. I still am not sure if ALL the centres do that (or if you have to pay fir that) but the appointment...

It doesn’t mention ILR either but the term 'settlement' visa I believe covers both FLR(M) and ILR. I guess we will see tomorrow or on the 5th when the UKVI visa application sites should explain...

Yes, those figures are given as an example and are for Tier 2 General. It does give us an idea of the scale of fees for the different levels of service.
