Really? Thanks for that. Now I'm worried! I cannot find reference to this anywhere on the ESTA FAQ but just to be sure I'll book a fully refundable one way ticket back to the UK from Canada.

I mean no, it's not because you have a medical card that you have not heard. It's because you are not at that stage yet.

Does anyone know how you show you have left the US when arriving on ESTA and driving back to Canada? There is no green slip to hand in.

They are only asking for hard copies if you require photos. I've also read elsewhere that in some cases they don't even require email copies - they go off the ones you sent on application.

You did not immigrate in 2012, it was 2011. Therefore you should not check yes to this box.
