Well here's what I would do...

I'd cancel with Air New Zealand and take the full refund, and then book with Qantas leaving from Rotorua airport. They have direct flights to ChCh at $116 each way...

Only thing I could advise is to ask to be listed for an earlier appointment (with the specialist) should a cancellation be made by someone else - your wife would need to be able to go within a days...
