You start all over again with fresh forms (make sure they are the latest versions). Make sure your police certs are either still in date (unlikely) or get new ones.

I am 65 living in London, ON. Got my first vaccine shot (Pfizer) today. Appointment set for 18 August for 2nd shot (some 3.5 months gap). They did tell me I should have 92% protection in 2 weeks.
