Like all international team or individual rankings they are based on results over the last x months or years - who you beat and their ranking. So if Wales do well against relatively weaker nations i.e. 6 nations, then it doesn't do a lot to boost their rankings. If however they get consistent wins against the top 3 or 4 then their ranking improves. The problem of course is that Wales don't play the Kiwis, Springboks or Wallabies often enough to improve their ranking drastically.
If the Lions had put out 15 Welshmen for all 3 tests against Australia they may have a good case for a higher ranking.
