I have now collected all the information needed to file for my interview at the US Embassy - it all took more time than I imagined, (including the Police Report etc) but I finally got it...

Sure does, asks for "all previous visits to or residence in the United States" FROM/TO (mm-yyyy).

I've got my old passport - will have to try to figure it out!

I have a multi-entry B1 10-year holiday Visa - I showed sufficient funds/reason to request a year at Homeland Security in SEA and they let me in for a year.
