No my wife didn't get this email and as BritInParis has advised, I wouldn't read too much into this email. A lot of people get it and still end up with a positive decision.

My wife got a request to pay the IHS for herself on November 22nd and got a decision made email on December 1st.

Regards to 2, I agree this is likely the case as I wouldn't expect they would ask...
