At least Arizona has a reciprocal agreement with Germany, but the staff of some DMV's may not know it.

I never denied that.
But when the licence expires, is lost or stolen, then a new licence can legally only be issued in the EU country of normal residence.

This principle is correct for all EU/EEA countries regardless if they print an address on the driving licence or not.
Read the 2nd EU driving licence directive, which is legally binding for all EU...

She does not have to take the written test and the driving test, when she exchanges her German license for a Maryland one, because Germany and Maryland have a reciprocal agreement for regular car...

As I already said:
France has not yet implemented the subcategories C1, C1E, D1, D1E in their driving licence model.
These categories must be implemented in all EU countries from 2013 on.

You do not loose any rights when converting your driving licence.
You get the rights according to the
Commission Decision on equivalences between categories of driving licenses:...
