The text of the notarial certificate ("subscribed and sworn to before me ...") is correct, though (at least in California) it would be better if the state and county were directly above that text.

With the caveat that I don't know what CIC will and will not accept, yes, I think you have it. The main point is that the notary (at least a California notary, which is what I am) CANNOT notarize...
