The form is used for individuals and businesses, so can be confusing.

Business Name - leave blank. It actually says "if different from above" which is your name.

Federal tax classification....

Just divide the GBP amount by the USD rate i.e. £10,500/0.786 = $13,358.78, so $13,359 for the FBAR.

They received my wife's application on March 23, 2023 and say that she can "expect a reply by 26 December, 2023", so the processing time has improved.

You can check progress on-line with a UK...

It is a UK mobile number which I needed for my UK bank accounts. I ported it from Virgin Mobile when they stopped PAYG.

I don't know about PayPal or other payment services. I use my US cell number...
