They did not ask that question when I had my medical done. They asked if I was on medication of any sort but no question about drugs. If you don't have a criminal record because of your drug use why...

I got the I-134 with my packet 3 (I am the foreigner) but you can also download it off the INS website. Will allow you to get it filled out quicker and saves postage.
