How to report a marriage fraud to INS?
What are the ways to report the immigration fraud to INS? What are
the results or actions INS taking after the report? Which way is the
most effective?

Damn, we've had this question posted so many times in recent months. Almost makes one think that maybe the service centers and consulates should slow down a bit, and be be a bit more demanding about evidence....

INS no longer exists. Talk to your friendly ICE representative. Make your report. Mayby they will do something, maybe they won't. If they do, then some deserviing immigration lawyer gets a removal defense case before the court.
BTW, I haven't lost one of those marriage fraud defenses yet. I like them and have a certain amount of fun -- which you don't get from an asylum case.

What are the actions the BCIS do after they receive the fraud report?

The ICE will probably do little unless you have UNSURMOUNTABLE proof of fraud.
Reason being, if the IS takes the couple to court, it's up to the IS to PROVE to the judge that the marraige is a sham. During the AOS it's up to the couple to PROVE that they are bona-fide marraige.
That's why most marraige cases always get approvd if you have enough money. Cuz even if the interview was totally denied -- if you got the funds -- get a lawyer and send it to court, where now the IS *Has* to prove to the judge it's a sham marraige. Which is difficult for them to do, because during that time, you can keep gathering up more proof of your marraige, and it winds up being impossible for them to prove you otherwise.

iron,
Not sarcastic, but I think you do get my drift. I should perhaps have limited my comment to the part about being more demanding about the evidence, and that would tend to make things slower than they already are....
Everybody is in such a rush to be united with their true love, but we are getting so many reminders these days that the love is sometimes only 1 way. Sad, but true.

Re: How to report a marriage fraud to INS?
I think the issue here is equal rights for all US citizens. The critical question is "why are we being forced to wait sometimes over 200 days for a Fiance VISA and to prove that we indeed love the person we are engaged to marry?". I think we have obviously seen that it is not for reasons of national security, since the security check does not take 150 days. If it should be imposed, as you suggest, to ensure that the love and intent is genuine... then this standard should be imposed upon all US citizens intending to marry. I'm sure that failed marriages to immigrants have far less effect on our society than failed marriages between US citizens. But, I think we all agree that the government should not be in the business of sanctioning or rejecting the validity of marriages. So, my point is, even if you think it's a good idea for the government to intervene and make sure these marriages are genuinely based on mutual love, we as US citizens should be free to make the same potentially stupid mistakes (or hopefully wonderful, lifelong attachments) as the rest of our countrymen.
I just needed to vent this morning, because obviously the only reason for the wait is inefficiency on the part of the government... no more, no less.

Marriage to an Amcit does not confer citizenship and thus "rights".
