I agree. All major legislation has needed fixes after first being passed. With the ACA, all we've had from Republicans are 60 odd attempts to repeal it.

That's because they think the ACA is creeping socialism and the creation of a President whose black and a secret Muslim. Ye God's what's the USA coming to?

I am not sure what the solution is, if there is one.

The local Surgery here closed, I understand that one of the main reasons was that the reimbursement rates were too low and the operating costs too high.

Medicaid I believe has the lowest reimbursement rate, seems the last operators wanted to provide extra 'services' and that presumably was part of their business plan, high margin but you need people who can, or whose insurers can pay for them.
