Get in touch with your credit card company if by DD and get them to sent you a declaration form if they will not cancel over the phone, good luck

If you have a UK address just ask for a card, not an install.

So I take it that the Sky cards as they are sent out are all identical and not matched to one specific box until such time as they are activated?

But if you order one you normally have to give them the sky box details when you order one so they have the details ready in the system for phone line activation....
The new white cards that are being sent out to replace you old blue yellow house subscription cards are already tied to your specific boxes - they cannot be activated in any other box unless you have a very long call with sky customer services.
