Odd as it sounds, I think moving actually helped us. :) My UK husband filed his AR-11, but I didn't know about the I-865. They helpfully mailed me and requested that I file it, so of course I did....

If worst comes to worst, there are some places near the embassy to have photos retaken, if need be.

I don't know, but I assume the envelope is to prevent tampering with the paperwork. If he can't get his paperwork out of the tear, it should be fine.

Don't know about Warsaw in particular, but I was the only USC there at the interview in London. As far as I know, it is not required anywhere.
