I used ozforex and I must say they were really helpful - they even offered to help when my own bank in the UK (Barclays) wouldnt transfer MY OWN MONEY to Australia

You can ring the surgery and organise it - explain its for Australian immigration purposes they will tell you whats documents to bring - usually passports and any glasses you may wear

The references dont need to say I declare...... - they just need to be references.
However every photocopy needs to be certified as a true copy of an original
