Gavita,

Correct, you only have to complete 1295 and 5707.

There is no fee to pay (you should see it says $0!), you already paid the admin fee when you got your WTN and paid the service fee.

The letter says to "Complete the Come to Canada questionnaire". Google that.

Afterwards you'll answer some questions and you'll be given a checklist code which you'll need to use as a reference...
