For family reasons I am returning to the UK after 4 months in the US on H1B, my former employer has offered to ship back any bulky personal items for me, which is a computer monitor, games...

Tell me about it, I had to ask our attorney lots of questions.

My H1B interview was stress free, sat down at 08:15 had both window interviews, paid DX Courier and walked out at 08:40.

I did my DS-160 last week :)

Put your estimated date of arrival and duration of stay in months that are within your visa end date.
Arrival address, I talked to my employer and we agreed...

I am off to L.A. next week and my wife and kids are staying in the UK for a while I sort things out.

You misread my last post, totally different job/company.

I resign from my UK MD/CEO job and become a US employee of a US company working in the US on a H1B doing a specialist job.
