These items are not required. The instruction says, "one or more of the following" E through J. You do not need them all.
As for affidavits, should you choose to submit any, there can be no template as each affidavit will be unique to the affiant based on that person's knowledge of your marital relationship. The I-130 instructions are clear as to certain specific information that must be included, the rest is up to the affiant to tell what they know.

Under documents required for the I-130 it does state the following is required:
I. Affidavits sworn to or affirmed by third parties having personal knowledge of the bona fides of the marital relationship. (Each affidavit must contain the full name and address, date and place of birth of the person making the affidavit, his or her relationship to the petitioner of beneficiary, if any, and complete information and details explaining how the person acquired his or her knowledge of your marriage); or
J. Any other relevant documentation to establish that there is an ongoing marital union.
