If you've already paid the lower fee then you shouldn't need to pay again. Don't fill out the payment details or you may be charged twice.

You don't need six months worth of documents as proof of address, just the latest ones. You'll need to pay the IHS before you can book an appointment.

If not presumably she was required to sit an IELTS exam or similar before being allowed...
