One I don't think has been mentioned is health cost sharing. Not for everyone, and often have restrictions on who can join and what they pay, but it can offer cheaper premiums. Just this Saturday I was talking to a family of 3 who are self employed in the same family business and made the move a couple years ago from a $1k p/month $10k deductible plan on the exchange to a few hundred per month with something like $2000 deductible. They have used that for 1 child birth and the woman is currently pregnant with another.
