Scripts are even cheaper in Mexico.

I had very good treatment in Sri Lanka.
I know of people who have gone to Mexico.

The following links may be a useful start to treatment abroad:
http://www.treatmentabroad.net/
http://www.privatehealth.co.uk/
There was a very useful article in either the NY Sunday Times or Sunday Times (UK) about Health tourism. South Africa offers a stay at a game reserve in conjuction with surgery (that's before the lion takes a bite out of you). Many countries as said to have specialities, teeth, heart etc and are tapping into the growing unease over US health costs. Some of the accommodation in hospitals is comparable to staying in a hotel, apparently. The doctors are often US or Europe trained. Thailand, India, Costa Rica, even Europe are mentioned. The idea is you take a holiday and get your bypass all in one package. Sounds wonderful to me.
Perhaps the industry in the US will wake up one day. In the mean time I'm not lining the pockets of the paranoid litigation industry anymore than I have to thank you very much.

and canada is a good option for a lot, even if they have to pay because it's a lot cheaper, the language is the same etc...and on the abortion front, can get it done at reasonable times, well in maine you can't abort after 3 weeks which is kind of pointless ain't it.

As promised, as a final follow up I can report back that the Lloyds cover is not valid for US residents. It is being written through a company called MNU, and is aimed at US citizens leaving the US, potential residents who haven't arrived in the US yet, and residents who are not eligible for normal US cover. A Mr John Costa is marketing the policies to the expat community and has a somewhat flexible approach to the qualifying rules, to which I personally would give a wide berth.
End of story!
