Hi everyone,

I'll start by apologising for being a time waster, as this had almost certainly been covered before but I cannot find it in any of my searches!

I'm applying for an employment based green card, preparing myself for the medical exam but need my immunisation history. I never got this before I moved over here and suspect they won't give it to me unless in person? Would they give it my mum?

Does anyone have any experience of this?

My guess is if they would give the information to a third party nominated by you, they would need you to sign a waiver to that effect. Maybe more helpful if you either call them and ask, or your mother goes in and asks.

I needed a copy of my daughters record and just called our old GP, spoke with the nurse gave her my daughters NI number. She asked me a few questions took my email address and said she would speak with the practice manager. Two days later I got an email with the info I needed.

I didn't need the full medical record I just needed some dates and vaccination info.

Hi there

I wouldn't worry too much if you can't get the records, when I had my medical the doctor just asked for antigen levels to be tested for the vacinations required. I did try and get a copy of the dates but when I phoned up the doctors office I was told they didn't keep record from that time anymore (late 60's)

I phoned my surgery. They sent them out within the week. Easy.

My US Dr. allowed me to fill in an immunization card here with the approximate dates of my childhood jabs and then she signed it for me....I did the same for the kids.
