Forms Question

The downloadable forms from formshome.com are they acceptable for use and submission to the ins? Reason i am asking is because they have the formshome.com logo on them.
If not can someone point me to downloadable and fillable of230 please?
Also is it ok to write app for approx on these forms where dates of education and places you have lived are to go if you cant remember the exact dates?

No they don't. Not when you print them! They have a "Print" button, too, right? That
won't print either.
Yes, their forms are plenty fine to use. (Unless it prints FormsHome on them! hahah!)
