Ds-156
Questions about the DS-156 and 156K forms.
10. National Identification Number
I take that would be my Social Insurance Number in Canada?
19. What is the purpose of your trip?
Marriage on K1? Marry my fiancee?
21. When do you intend to arrive in the US?
Month and Year
22. How long do you intend to stay in the US?
Permanently
But what if in a number of years I do decide to leave and take up residence in Canada after I obtain USC first.
23. Who will pay for your trip?
Self
24. Names and relationships of persons traveling with you
None
25. Do you intend to work in the US?
Yes, but I don't have a job there yet. Should I say that or just leave the US employer address blank?
27. Have you ever been in the US?
Should I just tell of the latest trip by plane even though it was a short trip? It was also while the I-129F was just being approved, would that pose a problem or should I choose some trip way in the past that I would be able to back up?
28. Have you ever been issued a US Visa?
As a Canadian, I don't need a visa to travel to the US unless I intend to work. I've never received any stamps in my passport so I assume this question is NO.
Thanks,
Ian

Re: Ds-156
Hi Ian.. i too had similar questions and I asked the UK US Embassy about my
responses. They were curt and not very forthcoming with their reply but in
essance this is what I asked...
>Regarding the 156
>Section 19 asks what is the purpose of the trip. I have tentativly put in
>"Part of the K1 Fiance Application" but is there something specific you
>require in that section?
>Section 22 asks the length of stay. Tenatively I have put in "permanent".
>Again is there something specific you prefer.
>Section 25 asks if I intend to work in the US. Should I tick the YES box at
>this point. There is a possibility of a Job offer pending at this point but
>nothing certain so no addresses are available. If the K1 is approved, then
>I
>will of course be looking for a job once in the US.
>Section 31 asks if anyone has filed an immigrant petition on my behalf. As
>my fiancee has just mailed her petition off to the Vermont service center,
>should this now be ticked YES?
>DS-0230 Pt II Section 29 asks for the name & address of the petitioner.
>Should this be My Fiancee in the US name or my name?
This was their reply
>You should answer the questions as you have indicated.
>Your fiancee is the petitioner. The 230-II should be completed hand
>carried to the visa.
Hope thats helps.. That is of course the London Embassy requirements.

Do remember that this is a form that is used world wide, so there will be questions that don't fit a particular country.
Question #10 --Many countries DO have national ID numbers. The US is an example of a country that does NOT have a national ID number, but the US social security number has become a "de facto" NID. [This is actually a controversial political issue -- IMHO, the SSN will remain de facto and most likely will not become an official NID]. My understanding is that Canada is the same. I think it would be safe to put in "SIN" followed by the number. It will either be a correct answer or mere surplusage.
Question #22 -- permanent is just fine. "Permanent" is not a straightjacket to future actions.
Question #25 -- you are intending to enter in a status which allows employment so "Yes" will cause no probelms. Since you have no US employer -- the answer is "none."
Question #27 -- do list the last trip. That is no problem. For prior trips you can put in "numerous visits between [year] and present, none execeeding "x" days. Again, remember the form is for world wide use and not just in Canada with it unigue border procedures.
Question #28 -- the answer is "no." The consulate knows you are Canadian and therefore knows you were visa exempt.
Good luck.

Re: Ds-156
An add-on to the DS-156 form...
Question 31: Has anyone ever filed an immigrant visa petition on your behalf? YES or NO
From what I understood, the K-1 is non-immigrant visa, but Pete's response says that it should be ticked off as YES but K-1 is not an immigrant visa... so should it be NO?

Hi,
I ticked 'No' on the form since the K-1 is a non-immigrant visa, but wrote
(except for current I-129F petition) right next to it.
It worked just fine for me.

q1]> Question 31: Has anyone ever filed an immigrant visa petition on your[/q1]

I put YES for this question, but added that it was in conjunction with an I129F petition. I'm not sure if either answer would hurt, as long as they know what visa you're applying for.

No, it would not. INS does not know or care about the SIN. The question
you raise has been asked here sevral times over the past year or more
with no answer. No one seems to know what a US National Indentification
number is. Leave it blank (this is what my wife did anyway. Some are
paranoid about leaving blanks on any form).
