Please somebody help!
Do I need to complete and send Medical Examination Form and Affidavit of Support, if I’m going to send Adjustment of status documents to New York local INS and my basis for eligibility is DV2002 Lottery?

The question of whether the medical exam results should be included with the I-485 is
particular to each INS district office. You must find out what INS New York wants
before filing. In your case, you are probably O.K. including it, because your
interview must occur in less than one year.
No formal Affidavit of Support is required for DV-based I-485s. A variety of evidence
can be included with the I-485, including letters verifying employment and bank
records. It does not matter much what you put into the I-485, as long as there is
something; what matters more is what fresh evidence is taken to the interview.
Because you are going to have to get INS New York to expedite your interview, I
suggest you hire a good immigration attorney to assist you with the process. Ask
around; there are a number of people who lost their chance at DV-based permanent
resident status because they could not get the INS district office to expedite their
interview to an early enough date. If you don't get permanent resident status by
09/30/02, you have lost your chance.
