Visitors Visa Petition Waiver

A friend of mine who is currently in West Africa won a DV Lottery in 1994 and was recently
denied visitors to enter the US for business. In 1994 he went to the US Embassy for the DV
interview claiming his relative's children as his children and was subsequently denied by
the embassy for misrepresentation.
He was recently approved for a visitor's visa but the US Embassy cancelled it having found
that he was rejected in 1994 for the misrepresentation during the DV processing. They
however advised him to file a waiver but weren't given title of the form nor given the
forms number.
I do not know as to which form he needs to file and is it possible to file this form here
in the US for him with an address outside the US?
