vaccination supplement - AOS
I'm trying to get set up with a physician for him / her to fill out my vaccination supplement so I can Adjust Status.
I do not have any records of vaccinations - any idea what the cost might be ?
I'm from the UK if that makes a difference.

I don't know about the cost, but you can save some money by having the vaccinations done by your own doctor under your insurance plan, if you have one. Once you have the proof of vaccinations, that needs to be taken to the civil surgeon so he can fill out the appropriate form.

One thing i learnt when i got the vaccinations done (as i did not have the records with me,) is that u can usually go to the local health authority and get the vaccinations done probably as cheaply as u can there. (The civil surgeon doc was actually the one who told me this and let me know where i should go if i was to do that). The other option is to do it with ur own doctor under ur health ins, if u have that. I ended up just doing them with the civil surgeon, but i could have saved money.

If you're from the UK your own local health authority should have your
records on file, even if you're not registered with a doctor. Register with
one and ask for your vaccination history. If there are any discrepancies
then your doctor (or nurse at the surgery) will be able to administer the
required vaccinations free of charge. Remember to tell them it's for
immigration purposes though as this makes a difference. For example, I
needed the MMR vaccination and because I needed it for immigration I didn't
have to pay for it.
