USCIS administrative error??

USCIS administrative error??
There are other threads about this, but no solid answers. The
instructions offer no help in just exactly what an "USCIS administrative
error" is. Here is the story.
When my wife went for her interview for AOS the office was asked about
changing my wife's last name to mine. His reply was, "No problem". I saw
him cross out her last name and write my last name on one of the forms
in our folder.
The PR Card arrived and of course the name change was not made.
Now, do I send in an I-90 with box D checked and no money to see what
happens? Will an infoPass help? I won't even ask about calling the
misinformation line. We all know what good that will do.
