In addition if you do not have the 40 quarters of Medicare payments via payroll - which the OP's mother will not have - then you also have to pay for Part A of Medicare. Which is an additional $400 per month on top of Part B, C and D

You do not have to be a US citizen to get Medicare, but you do have to have been a permanent resident for 5 years.

For many people "40 quarters" may actually be achieved in only 34 consecutive quarters (4th quarter year 1 to 1st quarter year 10), because unless your pay is low, you can accumulate 4 quarters of annual eligibility in only one calendar quarter. So if you are lucky enough to have a well paid seasonal job for one quarter each year you could accumulate 40 quarters in only 10 quarters (one per year) of actual work.
