Anyone know what to do if your re-entry permit is stolen? Do we apply for a duplicate and
if so what is the procedure?
Thanks for your help.

You submit Form I-131 to the same INS facility to which the first I-131 was submitted,
together with photos, fee, a copy of the I-485 receipt and an explanation of what
happened. If the I-485 was employment-based, you would be well-advised to attach a copy of
the I-140 approval as well.

Oops, I was thinking of advance parole. You would submit the I-131 to the INS Nebraska
Service Center with photos, fee, copy of the green card, copy of the stolen reentry permit
if you have it, and an explanation.
