IR-9003 Form

Hi Everyone.
Keely here........ I was just wondering if someone can help me out. Tom and I are going
through the forms we need to AOS from K-1 Visa. We have been downloading them from the INS
Website and printing them out. I wanted to check on something last week and called the
INS. The lady informed me we also needed the IR-9003 (IRS Form) Well you can't find that
form on the INS or IRS Websites....it's just not mentioned. So I was wondering if anyone
knows where we can get this. I called the INS again today and another lady informed me
that we might just have to get the whole application from the INS (you know, where you
phone up and order the forms) Which I think is highly silly when you can download them and
not to have this form avaliable is frustrating. So I was wanted to know if anyone knew
where you can get it? There is a website that you can go to and pay $20.00 for it.... Just
wanted to know what others did.

We are doing the same thing...you can find a fill-in and downloadable one on Alvena's site
under Fiance Visa Forms and on Jonathan's under Step 4.....

You should file two (nearly identical) pages.
