ETA documents, what to include?

Default ETA documents, what to include?

A few days ago, I got my labor approved (took them 6 months) and received the documents in the mail. The package has 16 pages ETA forms that my manager filed online and a new page letter with my name, ETA case number etc and the signature of the certifying officer.

My question is, do we need to include all the pages that came in the mail or just the summary page (the new page) when sending I-140 and I-485?

If we need to include all the pages, does my manager need to sign the document as there is no signature of him on the form (page 9, right above the signature of certifying officer). Thanks.
