Confused?!?
Ok now I am confused about this whole vaccination thing. My husband did
have a physical before he came. But I thought we needed to go to a doctor to
get a vaccine supplement. Could someone please describe what exactly it is
he needs so I know what I am asking for when I talk to the doctor. He didn't
make a copy if the vaccination records from the doctor. He just handed them
in at the embassy and they put them in the brown envelope which he handed in
when we arrived. I mean if he has to get all of the vaccines all over again
that is fine, but I shouldn't have to pay for a whole physical and bloodwork
again when he doesn't need to show that.
Someone please help me!!! I am in the Boston area for anyone who didn't read
my previous message and maybe has some advice.
Thanks,
Lili Rossi

I guess your husband needs something called:
"Adjustment of Status Applicant's Documentation of Immunization"
where the doctor fills the immunization record - it contains
the list of vaccines (some of them are not necessary):
DT/DTP, Td, Polio, Measles, Mumps, Rubella, Hib, Hepatits B, Varicella, Pneumococal, Influenza
Quote from the BCIS web page:
I got mine from the INS office (together with the I-693 form),
maybe it's also available on the net (it's just one page, if I had
a clean one, I would have scanned it and posted)

Quote from the BCIS web page:
Who pays for the medical exam?
You do. You are responsible for paying all doctor and laboratory fees associated with your medical examination.
What form is needed for the medical exam?
If you are applying for adjustment of status in the United States, Form I-693, Medical Examination of Aliens Seeking Adjustment of Status, is used to report the results of the medical exam to the BCIS. If you are applying for a visa at a U.S. Consular post overseas, Form DS 2053 (Formerly Form OF 157) will be given to you. The Consular officer will supply this form and the accompanying supplements, and will give you the names and telephone numbers of Panel Physicians in your area. There is also a vaccination supplement which your Civil Surgeon or Panel Physician will provide. It is used to record the results of the vaccination assessment.

Re: Confused?!?
Vaccinations *L* I had fun with that one. I don't know where your husband is
from and that might make a difference. I'm from the Netherlands and when I
went for my physical I couldn't find my vaccination booklet, so I got them
all again in the Netherlands. The proof of that went in the brown envelope.
Then at my AOS interview, it was lost and I was told I had to get them again
here. So I'd say just wait and see, they'll tell you if you have to get them
again. Unfortunatly you will have to pay if he has to get them
again......atleast I did.
