Biographic Information
Hi
I just recieved all my forms to start going through the DCF process in
Sydney.
I have a few questions about the Biographic Information form, which I
believe is the first form I need to send back.
1. Do I use my maiden name or married name ( I won't be married until
Feb)
2. In my package I recieved two Biographic forms, does my soon to be
spouse need to fill one out also, or did they just give me an extra.

Dani,
You (or your fiance, whichever of you is the USC) can't do DCF until after you are married, so that should tell you what information to put on the forms... In places whee a maiden name rather than a married name is required, it will say so.
If what you are calling the Biographic Information is the 4-part G-325A, then yes one is for you and one is for your husband.

1.) Use the name that is current on the date that you fill out the forms.
2.) Your finance will need to fill out the Bio form for himself also.
