The pipe connection is probably still BSP - and the Americans use that too. Its probably 1/2" BSP or 1/2" iron.

Yes, I'm pretty sure that EVERY house I lived in in the US (10+ of them) had an attic hatch with sliding ladder. That was in MA, GA, TN and NC.

My experience of power showers in the UK was terrible. Not having them in the US was one of a myriad minor pluses I found when I moved here. My parents had one installed and it wasn't capable of producing more than a pathetic squirt of warm water in the winter, and in the summer the water was way too hot on the high power setting, .... and was only capable of producing a pathetic squirt of warm water on the low setting. My mother likes it, perhaps because it is much better than what they had before (which was no shower at all), and I guess she knows that this is as good as it is going to get.

Every house I have seen (NY, VA, & NC) in the US built since at least as far back as around, 1960, has a hatch with a fold-down ladder. Older ones usually have a small "British style" push-up hatch. Some I have seen that were built 90-100 years ago have NO access to the attic space!

UK is rapidly moving to mains pressure DHW anyway, so powershowers will die out in a few years.

I miss this too. But as I have a septic system now in the US, I might not be able to use thick bleach (not as much as I did in the UK) anyway.... Finding decent cleansers entails a bit of a learning curve in this country.

NPT is not the same as BSP.

There is a 5 degree difference in the thread taper, BSP being a 55* and NPT at a 60* taper. They will both fit into each other but you will have issues getting them to seal. Personally I wont risk a leak ruining my walls just to get a power shower! (but then I have my own well and just turn the pressure up on my pressure switch at the pump!)

These things don't seal on the threads, they seal on a rubber washer.

Got to depend on location. I've had some appalling showers here. Current place isn't to bad if we use the mist setting but the "power" setting is a bit limp.

Bus stops, with benches.....and regular buses for that matter, but when there are bus stops, there's hardly any cover and no seating....or bus laybys, they just stop on the road blocking traffic.

Take the head off.

See what your water flow is.

Take head apart, clean it out, if you are in a hard water area this can make a major difference. I soak the bits in Lime scale remover and give them a good going over with an old toothbrush.

If your flow is low, is it low to the rest of the house, is there a valve that is partly closed?
