She's had cosmetic surgery and is waiting until the bruising and swelling subsides before appearing in public.

It is puzzling although not big on my list of world interests.

If she has needed a kidney procedure and recovery is not going well or is taking longer than expected then it seems best that the WH issue a simply statement to this effect. Speculation will still happen of course but at least it goes partway towards mitigating that.
