Not many people can choose their location (though I have considered quitting AZ, maybe there's a patch for it).

But if it is coded as part of a 'procedure' (sorry, I'm not getting the right terms), it is no longer 'preventive' and has to be covered differently. This is not related to ACA at all.
Prior to the ACA, preventive colonoscopies (plus a bunch of other preventive services) didn't have to be covered as "free at the point of service". The ACA mandated that coverage.

Prior to ACA, insurance companies were able to refuse coverage based on pre-existing conditions, or set astronomical rates for said coverage (for private plans only).
An absolutely huge advantage of the ACA, as is coverage for U26 children on their parents' policy and expanded Medicaid (in those states that decided not to play politics on it). Could the ACA be improved? Of course, but it sure is a lot better than the situation pre-ACA and has helped reduce the rolls of the uninsured considerably.

Oh, and what Trump is proposing is a joke that will assuredly increase the number of uninsureds.

So Trump's response to Mitten's diatribe is that "'I could have told Mitt Romney to drop to his knees". Is he saying what I think he's saying? Because if he is then that's unfortunate as the evangelicals don't like that sort of thing.

Also Mittens is hoping a brokered convention and his stepping in and having another crack...

If I were Trump I'd play the recording of Romney's speech given at the GOP fund raiser three years ago.

I'm just loving seeing the Republicans tearing each other apart.

Debate is supposed to be online for free (Fox News).

I can't get a stream (without having to enter cable provider login details) - anyone else able to watch online for free?

When I look at Cruz, he reminds me of a Clanger.

Or is it another children's TV/cartoon character?
