Accident report form in English!

I found a link on the site a while ago for a copy of the Declaracion Amistosa de accidente in English so it would be easier to complete but cannot find it again now!
Could anyone please provide a link to an English version?

not sure if there is one on here, your insurance company should provide you with one if you request it
