I believe it was the secretary ne who made the mistake and forgot to include a form that I signed the first time and he fired her recently. If I had to guess I bet she messed up a few applications.

Thanks Rete I just spoke to my lawyer and I have the EAD form in front of me now so we will just apply this way and I am sure in 3 or 4 months they will just change the name at that point.
