Try a Google search for 'ziektekostenverzekering'. You'll get a list of insurers who specialise in sickness insurance.
Your basic insurance will cost you about €95 per month.

You're about right with the Amsterdam-Gorinchem commuting times. Rotterdam is probably better for you as you're only about 15 minutes away from Gorinchem.
