I just booked my medical in Manchester. The prices as of yesterday (28th March 2011) are:

All aged 11 years and over Â£120
Children younger than 11 Â£65
Chest x-ray Â£75

I just got the letter too. Im stunned; I only applied on 6th Dec 2010. When did you apply?

I see no reason to question it. They are just trying to get you organised so that the visa goes...

Same thing happened to me.

I just wrote my credit card details at the end of the application form you have to sign and send with a brief explanation of what happened. You can then pay in Aussie...

On the new NSW SMP plan, it states that they will sponsor people with 261313 (Software Engineer) who specialize in Java.
