I first rode Eurostar in 1996 and it was my understanidng then
that any Eurostar journey must be across the channel.

I didn't actually witness the incident. It was more of "Where's
Herb?" "He said something about having a cigarette..." "Ohmigosh!"
And it served him bloody right.

Why didn't he smoke on the train? Have they stopped smoking cars already?

Passports are supposed to be checked upon leaving the Schengen area. UK
immigration is very keen on checking all passports of incoming travellers.
And if they will not check your passport, check-in personnel of the Channel
Tunnel (if you travel by car) or Eurostar (train) will, as they, as a
carrier, are liable to heavy fines (thousands of pounds per case) if they
are caught carrying passengers into the UK without proper and valid travel
documents.
