PT permanent resident card application process

Your input if you have actual experience on this will be appreciated.
TLDR: application timeframe and information required for the PT permanent resident card.
We applied and received the "Certificado de Registo De Cidadao da Uniao Europeia" in late 2019 which we replaced as part of the Withdrawal Agreement with "Titulo de Residencia" (the biometric card) in 2022. After residing in Portugal for 5 years, we now look to apply for the Portugal permanent resident card later on this year.
The issues we have are:
1) what support documents are needed.
2) when can we apply (that is, how many days before the current residency period expires).
3) which authority do we have to make the appointment to begin the process, as SEF no longer exist.
4) the biometric card itself had a 2027 expiry (5yrs from the issue year in 2022). Which date should we follow, 2024 (from 2019 on the original application) or 2027 (from 2022 on the biometric card)?
5) is there a webpage that details all these information?

1) same as EU citizen requesting same permit
2) legaly 90 days before status change is due, but you got zero chance arranging appointment

aima desk officials have zero training about WA rights and will refuse to process your perm permit request. Their training and knowledge is limited to national immigration law.
