Default Employment Statement

Hi, hope someone can help. Gathering paperwork together.... ANMC asking for employment statement stating dates of employment over last 5 years. It says "if your dates of employment are not on your professional reference (which they are), and/or you have worked for diff employers within the last 5 years, you will need to provide a statement that details the dates of employ..as a reg nurse" This is to come from HR or Payroll.
Right, i have worked for my Trust bang on 5 years so ok there, however on the checklist for app form you have to supply one.

Common sense is telling me i dont need one....however, my head is pickled at the mo and i want to get it right. So, for all you lucky folks that have been thru this, what do i do??
