Hi there,

Im hoping that the whole affidavit form is ok for my co-sponsor to
fill in, as Ive come across a form which is just 4 pages long, which
also looks suitable for a co-sponsor to fill out.

Are they both ok?

They, confusingly both seem to be I134s?

Lee, sponsors and co-sponsors use copies of the same form for the I-134 and I-864. They tick different boxes to show primary or co-sponsorship.
