Some questions on EAD form.

Some questions on EAD form.
Hi all,
My wife is applying for her renewal EAD. We had a couple of questions on the form:
1. Question 10 asks for an A#. So far we have received 2 different A#'s. One is on the I-485 application/receipt and the other A# is on her original EAD card. Which A# do we put here? Should I go ahead and put both and put a note next to each?
2. Question #11 asks about previous EAD card and asks which INS office did we apply at last time and what as the date.
LAst time I sent the application to Chicago, IL because of the LIFE-K3 special address. But I beleive that the approval came from the Missouri Service Center. So which one would I put here? Chicago, or missouri? Also... is the date they want the date we applied? Date it was mailed? Or the date it was approved?
3. Does check get made payable to "INS" or "BCIS" ?
