I-94 question

Hello everybody,
Here is my story: I received my H1B and corresponding I-94 card about 4 weeks
ago. However, the letter containing my wife's and son's I-94's "was returned as
undeliverable" - that's the message on INS's answering system. According to them,
we should apply again, indicating the case number on the application. Before I go
into the pain trying to reach INS over the phone, maybe somebody who has been in
a similar situation could provide me some helpfull answers to my questions:
1) was there any status violation from our side (I woud assume no, but it's better
to be sure).
2) do we have to submit the whole application again, including the fee, or it is
enough to file a comleted I-539 plus a letter of explanation. Thanks in advance -
the info I get through this group is usually very useful and accurate.
